How this form works right now
To protect student information, this form does not transmit or store anything online yet. When you press “Prepare request,” we assemble your answers into an email you send from your own address to the Records office. Uploaded documents are not sent or saved — you will be asked to provide them separately through a secure method the office confirms with you. Secure online submission will be enabled once approved.
